Your Sims have question marks on their bodies due to incompatible or broken custom content (CC) files. The game can’t read or retrieve texture data from these files, which can also cause body parts to disappear.
Fixing Question Marks on Sims
Remove All CC and Mods
CC and mods can sometimes break the game. Remove all CC and mods, restart the game, and check if the question marks are gone.
After moving CC and mods to your desktop and restarting the game, your Sims should return to normal, but without clothes.
Next, close the game and move the mods back into the original folder, except for the Resource.cfg file. Check if the issue is fixed.
After you remove or update your custom content, delete the localthumbcache.package file from your The Sims 4 folder.
You can find it under: Documents\ElectronicArts\TheSims 4\localthumbcache.package.
This file saves cached data, and sometimes it can keep causing problems even after the broken CC is gone.
Deleting it helps clear out leftover data and can fix lingering issues.
Always make sure all your CC is compatible with the game. Many users suspect 18th-century CC clothing might be the problem. If you use this type of CC, remove it first and see if it helps.
Repair the Game
Repairing game files can also help. Open the EA App, go to your Game Library, select The Sims 4, click on More Options (the three dots), and select Repair.
If the problem persists, try repairing the game multiple times. Often, the third repair is sufficient.
Change the Skin Tone
For a specific Sim, you can try changing their skin tone to something else and then switching it back. This can refresh the Sim and sometimes fix visual or skin-related glitches.
If you’re using mods that include adult skins or textures, you’ll need to turn off Laptop Mode and turn on Uncompressed Sim Textures in your graphics settings.
This can also apply to some custom content skins, since those options affect how detailed skin textures are displayed.
